I am writing to urgently report a serious incident regarding the safety and welfare of an elderly resident named [Resident Name], who resides at the [Facility Name] in [Location].
On Tuesday, 26 May 2026, at approximately 7:10 AM, my partner and I were driving near a [Local Park/Landmark] when we observed an elderly man sheltering under a tree in the rain. We recognised him as a resident of the nearby aged care facility. We immediately turned our vehicle around to check on his welfare.
When we approached him, [Resident Name] was flustered, out of breath, and clearly distressed. We assisted him across the road and into our vehicle to get him out of the weather. When asked how we could help, he was adamant in requesting a lift directly to the [Local Police Station].

He stated that he felt "locked up" and had not been permitted to sign himself out for a walk in over a year, despite confirming he is not a resident of the dementia ward. To ensure he was comfortable and safe, I provided him with a drink and took a brief detour by the river to help him calm down before proceeding to the station as he requested.
We arrived at the [Local Police Station] at approximately 7:40 AM. The station was not yet open, so I utilised the external police link phone to summon officers, waiting with [Resident Name] for 10–15 minutes. After assisting him to the restroom, I left him in the care of the attending officers.
I am raising this matter with the Commission for three primary reasons:
Safety and Supervision: It is deeply concerning that a resident was able to leave the facility unnoticed, walk a significant distance, and be exposed to the elements without staff being aware of his absence.
Resident Rights and Liberty: [Resident Name]'s statements about being forcibly confined and denied the dignity of a simple walk warrant an immediate review of the facility's adherence to the Charter of Aged Care Rights.
Falsification of Records: I have strong reason to believe that the facility's internal progress notes regarding this incident have been fabricated to downplay the severity of the resident's escape, falsely claiming he was intercepted just "out the front" of the facility.
Evidence Available:
I hold date-and-time-stamped video evidence of [Resident Name] off-site, away from the facility, lucidly explaining his situation and distress. I am prepared to provide this raw video file directly to an investigator via a secure portal.

So I called when I woke up around lunch dialed them on the stagger for a morning piss.
Was surprised they picked up after 30 seconds, but had that point clarified; heck even had a call back around 1pm from the inspector assigned to the case.
So blow that whistle now for repercussions.
